writing a SMART Goal for my community issue:

 

Provide monthly Free community “Educational Clinics” held by the county health clinic to teach about diabetes, nutrition, and to provide resources for those without health insurance or access to primary care such as interpretive services to assist people with insurance enrollment process. The teachings and information will be provided in Spanish. This would help address my community health issue of Obesity in the adult Latino community.
