Great post! You made several good conclusions regarding the symptoms the patient is experiencing. She could be experiencing confusion due to the infection, and she may be suffering from sepsis. For the Glasgow Coma Scale, I gave her an overall score of 12. She received three points for her eye response, four points for confusion, and five points for movement with painful stimuli. Remember to include all parts of the nursing diagnosis statement. The health promotion and problem focused diagnoses need to have the related to and as evidenced by portions. The risk diagnosis needs the as evidenced by portion. You do not need to include related to in the risk diagnosis. Writing correct and applicable nursing diagnoses is an essential part of the patient’s nursing care plan. You will gain more experience with nursing diagnoses throughout your nursing career.
